Central City Concern

Central City Concern is a non-profit organization whose mission is to provide comprehensive solutions to ending homelessness and achieving self-sufficiency. CCC provides many services, such as addiction treatment, employment, housing, mental health, primary care, and other related culturally specific services, to a diverse population. TITLE: Summit Care Coordinator
DEPARTMENT: Old Town Clinic
REPORTS TO: (Summit) Care Team Manager
SALARY: DOE
SCHEDULE: Monday-Friday 8:45am-5:15pm

SUMMARY: The Ambulatory Intensive Care Unit (Summit) Care Coordinator (MA) functions as the central member of the clinic patient team as both the medical assistant and health coach, directed by the Summit Team Manager. The Summit team is not an acute care team, but a highly trained, integrated primary care team supporting patients with advanced illness. The Summit team is part of Old Town Clinic (OTC), a patient-centered primary care clinic providing integrated behavioral and medical health care to homeless and low-income patients in downtown Portland. The Care Coordinator acts as the key point of contact for patients managing their chronic conditions and advises patients on goal setting and action plans as the primary focus of managing their disease. Works directly with patients and their caregivers to monitor and coordinate their care while assisting in navigating the healthcare system. Identifies and facilitates completion of preventative and follow-up care as it relates to assigned patients.

MINIMUM QUALIFICATIONS:
1. High School or GED required.
2. Completion of a Certificate-level Medical Assistant program accredited by the Commission on Accreditation of Allied Health Educational programs (CAAHEP).
3. At least one (1) year recent, verifiable medical assistant experience within a health care setting preferred.
4. Certified Medical Assistants preferred.
5. Bilingual (Spanish) preferred.
6. Phlebotomy experience preferred.
7. Must have current CPR certification or must be able to obtain in one month.
8. If a recovering chemically dependent person, must be in a program of recovery and able to provide demonstration of sobriety for a minimum of two years.
9. Must possess a current drivers license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
10. Must have the ability to read labels and distinguish between colors.
11. Must be physically able to walk, bend, stoop, be on feet for long periods of time, twist, kneel, reach, squat, pull, lift heavy patients or objects and climb stairs several times a day.
12. Must pass a pre-employment drug screen, TB test, and background check.
13. Must adhere to agency’s non-discrimination policies.
14. Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity.
