Position Description: Community Health Worker
Pay Range begins at $33,280 + Current Array of Benefits
SUPERVISOR: Community Health Programs Manager Proposed Start Date: ASAP
SCOPE: The Community Health Worker provides a variety of health, stability and wellness-focused services to individuals who have self-identified as desiring peer supports. The Community Health Worker should work to build individual and community capacity to improve health outcomes and eliminated disparities; reduce costs and barriers to good health; and create a more culturally competent and diverse healthcare workforce. Services provided by Community Health Worker should include a wide range of supports, services, and advocacy that contribute to participant’s ability to engage in ongoing treatment.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide supportive services to African Americans and others who are experiencing chronic disease in a culturally-responsive, empowering, and non-judgmental manner.
Assist Individuals to find and connect to the health and social services they need across the community.
Promote cultural mediation between program participants and the health care system.
Provide culturally appropriate and accessible health education and information.
Keep detailed and confidential records including reporting and intake documentation.
Provide informal counseling and social support to community members while advocating for individuals on a personal and community level.
Work within existing community groups to identify and address the underlying causes of chronic disease disparities while involving community members in a wide range of policy interventions to reduce chronic disease.
As needed, participate in health education workshop series’, organizing on-going education and support groups in the community.
Serve as a participant advocate, provide participant information and provide support for participants during emergencies as well as in outpatient, inpatient and mental health diversion settings.
Maintain caseload (up to 20 individuals). Monitor and keep case records on clients and report progress to supervisors; participate in well checks, retention calls, and other check-ins as assigned.
Maintain all records in accordance with the standards established Urban League standards, as well as all local, state, and federal laws and accreditation guidelines. Assure that all work is properly documented in participant records, or elsewhere, as required.
Perform quality assurance activities as assigned.
Assist in transporting and attending with participants internal and community based meetings as needed.
Provide participants with information about community resources and keep current on referral resources.
Perform other related duties as assigned.
EDUCATION AND WORK EXPERIENCE AND OTHER REQUIREMENTS:
High School Diploma or GED , Required
Bachelor’s degree in Behavioral or Health Sciences fields or a combination of at least three year’s work, education, training and/or experience in public health, Preferred
Community Health Worker Certification (or ability to complete certification process), Required.
Must have personal experience, membership in, and/or a close relationship to the community of African American with chronic health conditions.
Strong knowledge of local community health services.
Must possess and maintain a valid driver’s license and own mode of transportation at the time of hire and throughout the course of employment.
KNOWLEDGE, SKILLS, AND ABILITY REQUIREMENTS:
Knowledge of local community health services, community agencies and support groups.
Knowledge of public transit system preferred.
Proficiency in the operation of a personal computer including standard business applications Microsoft office suite including touch screen mobile devices and standard office equipment (copy, fax, scan).
Ability to facilitate individual and group meetings.
Ability to work independently.
Ability to communicate effectively in both oral and written manner.
Ability to establish and maintain professional relationships with community members, participants, co-workers, supervisors, participant family members and the general public.
Ability to handle and maintain confidential information in conformance with HIPAA guidelines.
Ability to work varied days and hours including evenings and weekends as needed.
