Macdonald Residence

The Macdonald Residence Assisted Living, located in the Oldtown neighborhood of downtown Portland has an opening for a Full Time RN. The Macdonald Residence is a 54 unit ALF serving our population alongside local nonprofit, Maybelle Center for Community www.maybellecenter.org. For the past 25 years, we have worked to help break isolation in this community. The Macdonald Residence is licensed by the State of Oregon and through our Specific Needs Contract we serve individuals whom often have a difficult time being accepted into other community-based care facilities.

Duties Include:

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned from time to time.

-Assist in managing all aspects of the nursing department.

-Performs new assisted living resident admission assessments prior to admission and determines appropriateness of the admission based on the needs of the resident.

-Performs a full body assessment on each assisted living resident upon admission within 7 days of admit.

-Performs on-going assessments of each assisted living residents physical and psycho-social needs and coordinates with dietary, activities, pharmacy, physicians and other disciplines to assure quality care.

-Assists with the self-medication assessment system in accordance with state guidelines.

-Assists the quality assurance program within the facility.

-Performs care conferences for residents and families as needed.

-Acts as a liaison between assisted living resident, family, and physician to coordinate the care needs of each resident.

-Assists with the maintenance of assisted living resident’s charts and reviews documentation performed by staff.

-Responds to urgent situations as they arise based on nursing standards.

-Monitor’s the weights of each assisted living resident and identifies significant weight losses.

-Assesses each resident with significant weight loss at least once per month until resolved.

-Assist in ensuring that all staff receives their T.B. testing per state guidelines.

-Assist in ensuring that each staff member receives their fundamentals of care giving, first aid, CPR, and HIV training per state guidelines.

-Will assess residents with skin breakdown or skin at risk every 2 weeks.

-Will be responsible for following the regulations and guidelines as set forth by the state of Oregon.

COMPANY WIDE RESPONSIBILITIES

-Supports a dignified and caring atmosphere with residents, residents’ families, visitors and staff.

-Protects privacy and confidentiality of information pertaining to the resident, employee, residence, company information and records.

-Maintains a safe and secure working environment and practices safe working habits.

-Complies with the attendance policy.

-Maintains neat appearance, good personal hygiene and appropriate attire.

-Participates in training, in services, and attends meetings as required.

-Assists with projects as assigned and performs other related duties.

SUPERVISORY RESPONSIBILITIES

Shares responsibility for care staff, leadership qualities are desired.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Must be Oregon Certified RN . Must have a minimum of 1 year experience working with geriatric residents–Bonus points if you have worked in the field of Behavioural Health.
